I. Consumer Eligibility:
A consumer will receive a comprehensive assessment to ascertain the bio-psycho-social issues surrounding the presenting problem. This will include a consult/assessment with the contracted physician who is authorized to prescribe *subaxone through a DEA or SAMSHA notification letter. Consumer participation in pharmacotherapy will require:
- a. the recommendation of the physician
- b. the recommendation of the substance abuse treatment team
- c. the commitment of the consumer to adhere to all recommended treatment activities.

II. Treatment Planning
The consumer will be assisted with the development of a person centered plan that includes, at a minimum:
- a. Group Therapy
- b. Individual counseling
- c. Regular medication checks with the physician
- d. Length of services should be no less than 6 months from initial treatment plan signature.
III. Participation Agreement
The consumer will attend scheduled sessions with a 90% attendance average. This is required for observation and symptom management. Failure to adhere to this requirement may result in termination of services and a referral to another provider for services. A contract will be constructed with the client and staff that illustrates the treatment plan activities and the agreement for consistent attendance. THIS MUST BE COMPLETED AND SIGNED PRIOR TO THE CONSUMER RECEIVEING THE PRESCRIPTION.
IV. Costs and Payment Agreement
Client will accept full responsibility for services rendered through a payment agreement. Consumers receiving public assistance (Medicaid, IPRS) will assist staff with authorization protocols through accurate disclosure of financial information. Payment plans will be closely monitored for timely receipts for services rendered. |
